If that's the case, then position restraint failure must be the reason behind the fall? 1

It's not as formal as RA and simpler, and it's possible that the person didn't check or didn't have his harness done up or the latch was only partially engaged and there was a sudden movement, mybe a gust of wind. Nothing most of us haven't done at some stage. 1
